When you receive an invitation to attend the program and agree the dates work for your family, we will schedule you to attend. 

There are a few things that need to happen BEFORE you receive your program confirmation:

• Your family confirms the program dates you will attend by completing this form confirming your acceptance and mailing it, along with check deposit, as directed.

• One or both parents will participate in a 30-minute phone call with one of our clinicians for an Intake Interview.  Provided here is the form we will ask you to complete, in advance, for the phone Intake Interview.   The purpose of gathering data from you is to help confirm that your child is a good fit for the program.

• You will be asked to complete these two forms authorizing the release of your child's medical information.  Please provide these to your primary care physician who will be asked to fax our medical team the information requested. 

• Several more forms and more information will be emailed to you during the two weeks prior to your arrival.  We also want to answer any lingering questions you may have.  Phone calls are always welcome!
